Provision of appropriate care pathway for the unwell baby at birth or soon after birth.
Health Visiting
Transfer baby to postnatal ward or Transitional Care Ward.
Provide care in partnership with Paediatricians in accordance with relevant care pathway e.g for cold baby, low blood sugar, low birthweight etc.
Birth or soon after
Provision of appropriate specialist care pathway & co-ordination of care.

Transfer baby to neonatal unit NICU where condition very poor at birth or worsening over time.
Very ill baby severe condition.
Congenital / cardiac condition, respiratory distress, very premature, hypoxic ischaemic encephalopathy.
Discharge home prior to 28 days P/N midwife to visit, post 28 days referral to relevant community professionals.

Proactive referral for additional support as required if baby very sick or mother has previous or existing mental health issues.

Discharge home notification to GP, HV and other relevant professionals.

Receives notification of discharge and provides ongoing care for baby as required.

Utilise information from family health needs assessment to agree in partnership with parents the management, support and onward referral for any concerns in relation to childhood development.
- Monitor development closely in situations where there are premature or small babies or where the birth has been traumatic on the neonate or suspected medical condition.
- Assess parental expectations and signs of attachment
- Promotion of age appropriate development, play and stimulation.
- Promotion of accident prevention.
- Liaison, information sharing and communication with GP and/or other appropriate professionals and specialist services.
0 - 6 weeks
Core Purpose 1
Outreach home visiting family support.
Childrens Centres & EYFS
Needs led service and encompasses issues such:
- Promoting positive bonding - use of tools such as Brazelton NBO
- Targeted one:one infant massage – home based
- Support to access appropriate groups/specialist groups
- Liaison with other agencies and communicate effectively and share information.
Referrals where additional needs are presented for parenting support.
